First Responders and their families
Applications for a Service Dog
Please print and fill out the correct form. Once you have the application completed and all the necessary paperwork, please call us to set up and day and time for an interview. If we are temperament testing your own dog, we will do so at the same time as your interview. Please bring a COPY your dog's shot records to the interview.
Please DO NOT go out and pick your own puppy. Service dogs need to have a very special temperament. We also prefer to have pups where their parents have been health tested if at all possible. If you need a dog to be your service dog, we will help you to find one or we will be happy to find one for you.
PLEASE CALL US AT 337-353-6782 WHEN YOU HAVE COMPLETED YOUR APPLICATION AND HAVE THE NECESSARY PAPERWORK SO WE CAN SET UP AN APPOINTMENT. BRING YOUR COMPLETED APPLICATION AND PAPERWORK TO YOUR APPOINTMENT. PLEASE FEEL FREE TO CALL WITH ANY QUESTIONS.
Active Duty Military Members and Military Veterans
First Responders
Louisiana Law states the following are First Responders:

Peace officers: Such as police officers or sheriffs

Fire protection personnel: Such as firefighters, search and rescue, and technical rescue members

Emergency medical services personnel: Such as EMTs, paramedics, and emergency medical responders (EMRs)

Emergency response operators and dispatchers: Provide communication support services for agencies

Other emergency response personnel: Such as state agency essential workers and members of the Civil Air Patrol includes a commissioned police officer, sheriff, deputy sheriff, marshal, deputy marshal, correctional officer, constable, wildlife enforcement agent, and probation and parole officer, any licensed emergency medical services practitioner as defined by R.S. 40:1131, and any firefighter regularly employed by a fire department of any municipality, parish, or fire protection district of the state or any volunteer firefighter of the state.
