 In this section you will find the forms you are needing to apply for a service dog. Please be informed that we know some of the documents required to complete this application does take longer than expected. All applications are placed into files for the Trainers to look over. Applicants are placed in a file, once all of the forms are completed you are called in to speak to our Trainers. If you are bringing your own dog:
 We will require that you bring your dog on the day of the meeting, so we may access your dog with a temperament testing. 


The Wounded Warrior application is for active duty military members, military veterans, or first responders with PTSD or TBI and other qualifying disabilities.  











The general application is for dependents and immediate family members of Active duty military or Veterans.  Please choose this form if you are interested in applying as a spouse with PTSD/TBI or other qualifying disabilities, or for a dependent child with special needs.
 
 
 
 




The Civilian Application is for people who are outside of the military. Please choose this form if you are interested in applying for you or your family member with qualifying disabilities. 








The volunteer application is for those who are interested in assisting our organization through donation of time or services.
